29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces: a.) Terminal Ready Mix, Inc. located at 524 Colorado Ave. Lorain, OH; On or about May 8, 2016, the employer did not ensure the workplace was evaluated to determine if any spaces were permit-required confined spaces. Employees are required to enter the plant cement mixer to replace the liner and are exposed to hazards associated with the rotating drum and welding fumes.
