29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides and railings were not provided with a toeboard where required: On or about 6/2/2016 on the mezzanine area in the equipment/chemical area of the facility located at 2434 West Broad Street in Athens, GA: Employees working and accessing the upper mezzanine area were exposed to a fall hazard of up to 11 feet 3 inches to the lower level from the section of the missing guardrail system for fall protection.
