29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part: Zee Construction Co., Inc. 4930 Woodville Rd, Northwood, Ohio: On or about May 14, 2016 the employer failed to ensure that a safe means of access and egress was provided for employees working in an excavation. An employee was struck by soil during a cave-in incident due to inadequate protective systems.
