29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed: (a) Employees were exposed to caught-in, struck by and amputation hazards, in that the employer did not conduct a periodic inspection of the energy control procedures for equipment, including but not limited to the CNC vertical machining centers, to ensure the procedures were correct and to ensure employees performing authorized work were performing the lock out and tag out procedures correctly.
