29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Fabrication department CNC Bender BD-01; On or about January 26, 2016 and at times prior thereto, the employer exposed employees to the hazardous energy (including electrical and pneumatic) present on the machine in that the employer did not require employees who change out the jigs on the bender to secure the hazardous energy sources. b) Fabrication department CNC Bender BD-03; On or about January 26, 2016 and at times prior thereto, the employer exposed employees to the hazardous energy (including electrical and pneumatic) present on the machine in that the employer did not require employees who change out the jigs on the bender to secure the hazardous energy sources. c) Fabrication department Endformer EF-01; On or about January 26, 2016 and at times prior thereto, the employer exposed employees to the hazardous energy (including electrical and pneumatic) present on the machine in that the employer did not require employees who change out the tubing on the endformer to secure the hazardous energy sources.
