29 CFR 1904.29(b)(3): Each recordable injury or illness were not recorded on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred: (a) USPS MPO Carr Annex - On or about February 11, 2016, the employer failed to record an injury/illness on the OSHA 300 and 301 forms when an employee suffered a strained shoulder while delivering a package. The injury resulted in days away from work followed by job restrictions upon the employee's return to work. (b) USPS MPO Carr Annex - On or about April 14, 2016, the employer failed to record an injury/illness on the OSHA 301 form when an employee was lifting a heavy package into her vehicle and strained her lower back. The injury resulted in days away from work. (c) USPS MPO Carr Annex - On or about February 11, 2016, the employer failed to record an injury/illness on the OSHA 300 and 301 forms when an employee was walking up the ramp and trays fell on her head. The injury resulted in days away from work. The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.29(b)(3), which was contained in OSHA inspection number 955929, citation number 1, item number1 and was affirmed as a final order on 3/21/14, with respect to a workplace located at 1606 Huron St., Ypsilanti, MI 48197. The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.29(b)(3), which was contained in OSHA inspection number 974326, citation number 2, item number 1 and was affirmed as a final order on July 24, 2014, with respect to a workplace located at 7451 Templhof Dr. Indianapolis, IN 46241. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
