29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) MCP 62 Circuit Disconnect (REC-2-9): On December 3, 2015, the employer failed to ensure lockout or tagout devices were affixed to the disconnect to isolate the energy to a conveyor gearbox that was being replaced.
