29 CFR 1926.950(c)(2)(i): The contract employer did not ensure that each of its employees was instructed in the hazardous conditions relevant to the employee's work that the contract employer is aware of as a result of information communicated to the contract employer by the host employer under paragraph (c)(1) of this section: a) 2998 Glenwood Drive, Camp Hill, Pa 17011 Substation - The employer failed to instruct the employees of energized electrical hazards in bay# 4 when work was assigned in bay# 5, an employee received an electrical shock when the aerial work platform he was working from came too close to an energized transformer in bay #4 on or about September, 28 2015. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
