29 CFR 1926.451(a)(6): Scaffolds were not constructed in accordance with the design by a qualified person. On or about 25 September 2015, four employees were working from a job-site made wooden trash box on the forks of a rough terrain, telescoping boom forklift up to approximately 19 feet high without the employer ensuring that the platform was designed by a qualified person, exposing the employees to a fall hazard.
