Text For Citation: 01 Item/Group: 001A Hazard: CRUSHING
"The following alleged violations have been grouped because they involve similar or related hazards that may increase the potential for injury resulting from an accident". Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: On or prior to 1/20/92, employee was exposed to the hazard of crushing injuries or death while operating Caterpillar 49G1135 Road Grader without rollover protective structure while clearing road on Inland Solon Sanitary Landfill site on the Southeast Haul Road. Abatement methods may include, but are not limited to, the following: 1. Fit all road graders with ROPS meeting SAE J396--1971 Standards and with seatbelts. 2. Inspect all vehicles prior to use to assure that they are fitted with ROPS and seatbelts as recommended in the industry and by U.S. Army Corps of Engineers EM-385-1 General Requirements. 3. Train employees using grader to avoid conditions that are potentially hazardous and which may cause rollover of the vehicle and also to wear seatbelts. 4. Restrict vehicle to surfaces involving grades and conditions not likely to cause overturning of the vehicle. 5. Document and enforce all steps of the above or of an equivalent program used to safeguard operator.
