Schulman Custom Compounding NE, Inc., 53 Millbrook Street, Worcester, Massachusetts; Procedures were not developed, documented, and utilized for the control of potentially hazardous energy for Smog-Hog #3 where an employee was injured on or about August 12, 2015. This plug and cord equipment does not meet the exceptions to this requirement as there is a potential for stored or residual energy (capacitors).
