29 CFR 1910.1030(f)(2)(i): Hepatitis B vaccination shall be made available after the employee has received the training required in paragraph (g)(2)(vii)(I) and within 10 working days of initial assignment to all employees who have occupational exposure unless the employee has previously received the complete hepatitis B vaccination series, antibody testing has revealed that the employee is immune, or the vaccine is contraindicated for medical reasons. a. Located at 100 East Broadway Street in Jackson, Ohio: On or about July 27, 2015, employees had an occupational exposure to blood while cleaning and sanitizing a forklift and the concrete floor located in Freezer 6 after a forklift accident. Employees involved in the task were not offered a Hepatitis B vaccination post exposure. 29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a. Located at 100 East Broadway Street in Jackson, Ohio: On or about August 21, 2015 and at times prior, employees were performing lockout on Extruder 1 located in the Fernandez Processing Plant in order to clean out build up within the screw drive compartment while processing, to clean and sanitize the equipment after processing, and occasionally to add oil to the mixing screw compartment. The employer did not ensure that employees were utilizing the documented lockout procedures and locking out the main screw disconnect, exposing employees to an amputation hazard. b. Located at 100 East Broadway Street in Jackson, Ohio: On or about August 21, 2015 and at times prior, employees were performing lockout on Extruder 15 located in the Goodwin Processing Plant in order to clean out build up within the screw drive compartment while processing, to clean and sanitize the equipment after processing, and occasionally to add oil to the mixing screw compartment.
