29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): a) BK Tires, Inc. at 3775 Puritan Way, Frederick, CO 80516: On or before July 16, 2015, the employer did not assess the workplace to determine what hazards were present that would require the use of PPE. Hazards included projectiles to the eyes from wheels, tire irons, and wrenches; the potential for rims to fall on employees' feet; and the chemical hazards of the Dowflake Xtra 83-87% Calcium Chloride Flakes used in preparing the lubricant for the tires. Condition exposed employees to physical and chemical hazards. Abatement note: 29 CFR 1910.132(d)(2) requires the employer to verify that a job hazard assessment has been done through a written certification.
