OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee(s) in that employee(s) were exposed to the hazard of being struck-by: a/ The last periodic inspection of a P & H 5T Underslung Crane (DL-8 South) did not include a thorough examination of the suspension pins. A suspension pin sheared off on or about July 02, 2015 resulting in the total collapse of the hoist and its suspended load. One method of abatement to correct this apparent hazard is to inspect for loose or missing fasteners, such as bolts, nuts, pins, or rivets as required by ANSI B11.30-2010, paragraph 11-2.1.5.
