The alleged violations below have been grouped because they involve similar or related hazards that may increase the potential for injury resulting from an accident. Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a recognized hazard of electrical shock and burns: (a) M/V Polar Queen: Employees working in the pipe room had occasion to work near a 110-volt outlet that has reversed polarity. This conditions exposed employees to electrical shock and burns if they came into contact with an ungrounded tool or electrical appliance using the outlet. Among other methods, one acceptable and feasible mean of abatement would be to properly install the outlet in ac- cordance with National Electrical Code standards.
