29 CFR 1910.119(d)(3)(i)(D): Process safety information pertaining to the equipment in the process did not include relief system design and design basis. The employer does not ensure that process safety information pertaining to the equipment in the process includes relief system design and design basis. The violation occurred on or about June 9th 2015 and at times prior thereto in the #4 anhydrous ammonia plant where the employer failed to document relief system design and design basis included but is not limited to the following: a. PSV-827/PSV-870 (P&ID PCC-230-0514. Rev. 2) b. PSV-88 (P&ID PCC-230-0514. Rev. 2) c. PSV-856 (P&ID PCC-230-0520 Rev. 2) d. PSV-710 (P&ID PCC-230-0523 Rev. 2) e. PSV-810/PSV-813 (P&ID PCC-230-0528 Rev. 2) f. PSV-860/PSV-897 (P&ID PCC-230-0532 Rev. 2) g. PSV-692 (P&ID PPC-230-0542 Rev. 2) This condition exposed employees to inhalation, fire, and explosion hazards. Pursuant to 29 C.F.R. 1903.19, within (10) calendar days of the abatement date the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that process safety information pertaining to the equipment in the process includes relief system design and design basis.
