Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: The following alleged violations have been grouped because they involve similar or related hazards that may increase the potential for injury resulting from an accident. a. Falls from elevations. On, or about May 13, 1992, two employees were working atop wooden formwork that they were stripping from over a 13 foot long by 8' 8" wide by 18 foot deep elevator shaft opening. The formwork collapsed causing an employee to fall 18 feet to the bottom of the shaft. Adequate fall protection systems were not provided to safely conduct the work. Such systems should include, but are not limited to: 1. Safety belts and lanyards with anchorage points for lanyard attachment. 2. Scaffolds or work platforms erected below the formwork from which the employees can work. 3. Any other industry recognized safety systems to protect employees from elevated fall hazards. The accident occurred at 1620 Woodall Rogers Freeway, Dallas, TX at the level P1 elevator shaft opening.
