29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about May 18th, 2015: Employees who performed servicing and/or maintenance on conveyors within a freezer induct station performed activities outside of the scope listed on the employer's Hazardous Energy Control Procedure number 61 and were exposed to caught-in, caught-between, and/or struck-by hazards from unexpected start-up and energizing of the conveyor system. The employer's Hazardous Energy Control Procedure number 61 did not clearly and specifically outline the scope to include servicing and/or maintenance activities such as, but not limited to, adjusting, inspecting, and servicing including unjamming. The general description for the procedure read: "MDC Conveyors, Electrical Only, Single Conveyor Switch Group, For Mechanical Repair Only." Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
