North Coast Plastics, Inc., 2114 Loveland Avenue, Erie, PA 16506, on or about February 20, 2015 and times prior thereto: Employees perform servicing and maintenance on equipment including, but not limited to, a Century Tech CT-9550 bag machine. The company does not have lockout tagout procedures.
