29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could not walk directly into the opening: (a) On or about January 16, 2015, and at times prior to that date, for employees exposed to an approximate seven foot fall hazard where the work platform on the Battenfeld-Fischer, Extrusion Blow Molding Machine, Type: FHB-054-1, Mach No. 4003, was accessed at 1311 Four Bottle Drive, Valley City, North Dakota.
