Text For Citation: 01 Item/Group: 001A Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about December 10, 1987, at the Blaine Construction Company site at 9th & Main, at the personnel elevator location, an employee was assisting another employee in the installation of a tie end rod, without locking out the equipment, exposing the employee to the hazard of the counter weight coming down on him as the elevator went up. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1) Institute a lock out system that requires equipment to be disconnected from all power sources and locked in that position prior to servicing or working on the equipment. 2) Train all employees in the procedure of locking out and the hazard if the procedure is not followed. 3) Locks keyed separately to ensure removal only by the employer who installed the lock to work on the piece of equipment.
