29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access: (a) On or about October 2, 2014, Santiago Roofing, did not ensure that portable ladders, used to access upper landing surfaces, extend at least 3 feet above the upper landing surface to which the ladders were used to gain access. Workers were observed using a ladder to access the roof of a residential building. Ladder did not extend at least 3 ft above the landing platform and was not tied to the roof edge.
