29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms: a) Building #2, Building #8, and Throughout Jobsite - On or about October 2, 2014 and times prior to, employees were working from pump jack scaffold platforms while wearing body harnesses with self- retracting vertical lifelines anchored to the pole of the scaffolds, resulting in fall hazards of up to approximately 22 feet.
