29 CFR 1910.184(c)(4): Slings shall not be loaded in excess of their rated capacities. On or about September 22, 2014 at the work site the working load limit of a welded steel chain was exceeded when the chain utilized for a lift of a blow-out preventer failed. Employees were exposed to struck by hazards.
