29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a) 1303 Hollis Ave, Cherry hill, NJ: A worker performing tree trimming activities from the elevated bucket of a Lift-All LAOC-51-1S was not utilizing fall protection. As such the worker fell from the basket approximately 55 feet to the ground below, on or about 9/16/2014. NOTE: IN ADDTION TO CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
