29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): The employer does not ensure that all energy isolating devices needed to control the energy to the machine or equipment are physically located and operated in such a manner as to isolate the machine or equipment from the energy sources. a) This violation occurred on or about August 21, 2014 and times prior thereto, in the ID Plant - Inbound Operator Control Panel where employees were exposed to struck-by and/or caught-in hazards when servicing and cleaning internal blasting equipment without all energy isolating devices needed to control energy to the machine or equipment being physically located and operated in a manner to the isolate the equipment from energy sources. b) This violation occurred on or about August 21, 2014 and times prior thereto, in the ID Plant - Blaster Operator Control Panel where employees were exposed to struck-by and/or caught-in hazards when servicing and cleaning internal blasting equipment without all energy isolating devices needed to control energy to the machine or equipment being physically located and operated in a manner to the isolate the equipment from energy sources. c) This violation occurred on or about August 21, 2014 and times prior thereto, in the ID Plant - Outbound Operator Control where employees were exposed to struck-by and/or caught-in hazards when servicing and cleaning internal blasting equipment without all energy isolating devices needed to control energy to the machine or equipment being physically located and operated in a manner to the isolate the equipment from energy sources. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is needed to control the energy to the machine or equipment are physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).
