Cardinal Health - 5995 Commerce Center Drive, Groveport, Ohio facility: a) The OSHA 300 Log for calendar year 2009, the one or two line injury (or illness) description did not include all the information required in column "F" for case numbers 0-90000199, 0-90000512, 0-90000818, 0-90000775, 0-90000952, 0-90001344, 0-90001613, 0-90002335, 0-90002976, 0-0910019529 and 0-90002964. The one or two line injury (or illness) description for column "F" must include a description of the injury, the parts of the body affected, and the object/substance that directly injured or made the employee ill.
