29 CFR 1910.124(j)(2)(i): The employer did not ensure airflow was adequate at least quarterly during operation: Employees were exposed to potential hydrogen gas buildup due to excessive foaming in the plating process and the employer had not ensured, at least quarterly, that during operation the air flow was adequate at chrome tank #26 in the plating department.
