29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident within 8 hours of occurrence. (a) Fremont Brakes & Oil LLC on or about July 24, 2014 at 03:50 pm (time of death), the employer did not report a workplace fatality within 8 hours after the death. The Toledo Area office was first made aware of the workplace fatality by way of media referral at approximately 8:00 am on July 25, 2014.
