Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of being struck-by air handling equipment being moved into place: a) TDC Project Fab 8 4th floor, on or about 6/26/2014, employees used Hilman Tri-Glide Rollers to move air handling units into their final installation locations. They did not use the rollers in accordance with the manufacturer's operating instructions, including by not using a spreader bar to ensure proper alignment of the rollers and stability of the load, and by not affixing the rollers to the load. Feasible and acceptable abatement methods to correct this hazard include, but are not limited to: * Comply with the Hilman Rollers Manufaturer's Operation and Installation Instructions which detail the proper use, installation, operation and maintenance of the rollers. * Use the spreader bar with the rollers to ensure proper alignment of the rollers and stability of the load. * Due to the limited contact surface area to rest on the rollers, secure the rollers to the unit. * Complete and submit a Hilman Rollers Application Analysis Form to the manufacturer for review, or otherwise contact the manufacturer for project-specific assistance with proper use of Hilman Rollers components. * The Employer may also connect the air handler units overhead to prevent tipping.
