1910.119(d)(3)(i)(F): Process safety information pertaining to the equipment in the process did not include design codes and standards employed. The employer does not ensure that process safety information pertaining to the equipment in the process includes design codes and standards employed. a. The violation occurred on or about June 18, 2014 and at times prior thereto in the MeadWestvaco chloride dioxide facility where employees were exposed to inhalation, fire, and explosion hazards when the employer failed to document the design codes and standards used in testing interlocks. b. The violation occurred on or about June 18, 2014 and at times prior thereto in the MeadWestvaco chloride dioxide facility where employees were exposed to inhalation, fire, and explosion hazards when the employer failed to document the design codes and standards used in the design of the ventilation system for the chloride dioxide plant building.
