Text For Citation: 01 Item/Group: 001 Hazard:
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm, to employees in that employees were exposed to the hazard of being crushed by an unsecured counterweight from a Toyota Fork Lift Truck. a) On or about June 17, 2014 - Jobsite at 58-58 56th Drive Maspeth, NY 11375 While lifting the rear of an out of service Toyota forklift using a second forklift, the counterweight of the out of service forklift , which was inadequately secured, became dislodged and fell, crushing the employee who was in the rear and at ground level attempting to finish the repairs. WRITTEN ABATEMENT DOCUMENTATION IS REQUIRED.
