29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment: (a) Slot 47 (5841) - On or about June 11, 2014 employees were exposed to electric shock hazards while working near a 480V disconnect box with a 1 and 3/16 inch hole in the sidewall. The hole was not effectively closed. (b) Maintenance Shop - On or about June 11, 2014 employees were exposed to electric shock hazards while working near a 480V to 240V transformer with 2 holes in the sidewall over 2 inches in diameter. The holes were not effectively closed.
