29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph. a) Jobsite: East scaffold - On or about 5/20/14, the supported tubular welded frame scaffold measuring approximately 33 feet in elevation and 5 feet in width, was not restrained from tipping or falling over by guying, tying, bracing, or equivalent means. An employee was conducting masonry operations from the 3rd working platform, approximately 20 feet above ground level. b) Jobsite: West scaffold - On or about 5/20/14, the supported tubular welded frame scaffold measuring approximately 26 feet in elevation and 5 feet in width, was not restrained from tipping or falling over by guying, tying, bracing, or equivalent means. An employee was conducting masonry operations from the 4th working platform, approximately 26 feet above the lower level.
