OSH Act of 1970 Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards from a hydraulic dock leveler: Location: 6300 Creek Rd, Blue Ash, OH. On or about April 30, 2014, at the perishable side of the warehouse at dock #45, employee(s) operating a Kelly model HV 615 hydraulic dock leveler were exposed to struck by hazard while standing in the pit to lower the ramp and was struck-by and crushed by the ramp during the activation of the dock leveler.
