29 CFR 1926.1408(a)(1): The employer did not identify the work zone where crane/derrick operations would be conducted as either demarcating boundaries and prohibiting the operator from working past those boundaries, or defining the work zone as the area 360 degrees around the equipment up to the equipment's maximum working radius. On 21 April 2014, and at times prior thereto, a rough terrain crane with a boom arm was allowed to operate near and under an energized power line, without either the work zone being established and defined, exposing the operator and co-workers nearby to an electrical hazard of approximately 25kV 3-phase voltage.
