29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: On or about April 16, 2014, at the above addressed site, an employee was exposed to potential head injuries when cutting fronds of a palm tree directly above his head while not wearing a protective helmet.
