OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: A.) Holbrook Tool & Molding, Inc. Dust Collector Room: On or about 3/20/14, employees were potentially exposed to fire and explosion hazards from dust collectors located inside. Dust collectors including but not limited to the Torit dust collectors (Serial #21849B; Serial #213490; both model 2DF8) were located inside the building and were not equipped with any devices to mitigate a potential dust explosion. The dust collectors collect combustible carbon dust. B.) Holbrook Tool & Molding, Inc. Torit Dust Collection System: On or about 3/20/14, employees were potentially exposed to fire and explosion hazards from dust collectors that were not equipped with devices and systems to prevent the propagation of explosions and associated flame fronts from the dust collector through the associated dust collection system duct work. Torit dust collectors; Serial #21849B; Serial #213490; both model 2DF8) were not equipped with approved devices to prevent the propagation of a dust explosion and associated flame fronts throughout the dust collection system. The dust collectors collect combustible carbon dust.
