29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about March 11, 2014 and prior thereto, the employer had not developed machine specific lockout tagout procedures for Mill #3 and Mill #2 to prevent the energization and movement of the bridle when employees were performing work such as but not limited to cleaning scrap from the mill when the bridle was open. This condition exposed employees to the hazard of being struck by the moving bridle or caught between the bridle and stationary objects such as the mill, bung or plumbing for the hydraulic system.
