29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about February 11, 2014, and at times prior thereto, lock out/ tag out procedures, effective training and periodic inspections were not established and in place for employees operating the RotoMix commercial cattle feed mixer when performing maintenance and clearing of contaminants such as plastic wrap. NOTE: Abatement certification AND documentation are required for this item.
