29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): a) Fabricated Scaffolding System located right of the residential structures front door: On or about January 22, 2014, the employer and/or a competent person did not ensure the 3rd level of the scaffolding system was fully decked before used which exposed an employee to a fall of 18-feet to the ground below.
