29 CFR 1910.176(g): Covers and/or guardrails were not provided to protect personnel from the hazards of open pits, tanks, vats, ditches, etc. On or about January 23, 2014, on the east side of the garbage and plastics pit in the wood yard area, the employer failed to ensure a cover or guardrails were installed around a sump pit that was 12 feet wide and 8 feet deep.
