29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm in that employees were exposed to struck by hazards: a. On or about 01/23/14, US Army Reserve-AMSA, Building 653, Route 28, GUAYNABO, PR - Yard /Dumpster Area: An employee handling an Automatic Fire Extinguishing System while cleaning out a warehouse was struck-by the cylinder when it discharged its contents and became projectile. Employee was exposed to struck by hazard cause by the inadvertent discharge of an unsecured fire extinguisher.
