29 CFR 1926.502(a)(2): The employer did not provide and install all fall protection systems required by this subpart for employee(s) and did not comply with all other pertinent requirements of this subpart before employee(s) began the work that necessitates the fall protection: On or about January 14, 2014, at building #52, 549 April Dr., Canyon Lake , TX, the employer failed to ensure employee(s) were protected from a fall hazard to the ground below. The guardrail system that was installed on each floor was missing top rails, mid rails and the top rails could not withstand a 200 pound force from an outward direction. Employee(s) were exposed to a fall hazard of approximately 29 feet to the ground below.
