29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Location - 33 Glen Avenue, Chelmsford, MA: On or about 12/18/2013, employees performing a setup operation on Machine #126 failed to utilize the company's established lock out tag out procedure for that operation. Packaging Corporation of America was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 314427618, citation number 1, item number 1 and was affirmed as a final order on 8/17/2010, with respect to a workplace located at 3200 HiPack Drive, Opelika, AL 36801.
