29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident within 8 hours. On or about 11/5/2013 the employer failed to report to OSHA a work related fatality within 8 hours of the incident. The employer reported the fatality ten days later on 11/15/2013.
