29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about November 06, 2013, and at times prior thereto, the employer did not provide or ensure the employee successfully completed the training, evaluation and certification required by paragraph (l) prior to operating the equipment for an employee who was operating a Case 621C front end loader with a forklift attachment.
