29 CFR 1910.333(c)(3): When work was to be performed near overhead lines, the lines were not de-energized and grounded, nor were other protective measures provided before work was started: a) 2805 Pierce St., Wall Twsp.: A worker was using a Stihl extendible pole saw to trim a tree from a ladder approximately 30 feet above the ground. The tree and working location were near energized primary and secondary overhead power lines. The pole saw contacted the primary overhead power line, electrocuting the worker. The lines were not de-energized and grounded, nor were other protective measures provided before work was started. Observed on or about 11/02/2013. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
