29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about October 7th, 2013, employees were not provided with personal fall protection systems while accessing the south plant conveyor system to change a belt on the B4 Conveyor system (Mayfran International serial number c7265-9 Mayfran 930651). Employees were required to step onto two machine guards, climb into the B5 Conveyor, and climb on top of the B4 conveyor to access the belt system. Total overall height of the belt sytstem to the ground level was measured as 16.91 feet.
