29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the equipment or machinery was not isolated from the energy source and rendered inoperative: (a) Detweiler Lumber, LLC, 46 Front Mountain Road, Allensville, PA - The employer did not establish an energy control program for the servicing, maintenance, or cleaning of equipment or machinery, such as but not limited to the head saw and log carriage, thus resulting in fatal injuries, on or about October 14, 2013. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
